嗯..那是因為你的CWM版本太舊導致刷機失敗了...
記得 CM12.1 Android 5.1.1 一定要 CWM 6.0.5.0 以上等才能安裝
或者刷 CM10.1(4.2.2) -->CM11(4.4KitKat)-->CM12.1 吧
樓主提供goo.im網站掛了好像不能下載那個 philz_touch_6.48.4-n7000.zip , 要看別人要不要幫忙補檔
所以你可能要找 raw kernel r10 來刷更新 CWM最新版本 6.0.5.0
刷機前應該先看這篇文章如下, CM12.1 ROM 該作者有寫說明...

CM12.1 N7000 刷機的方法有兩種
<A>
If you are running an custom Kitkat 4.4 Rom or an earlier NightOwl Beta:
1. Copy Rom + Google Apps to your device.
2. Boot into recovery
3a. Factory Reset (Recommended to avoid any FC or bootloops) (Updates from older NightOwl Betas should work without Factory reset)
3b. Wipe the dalvik-cache in recovery after updating from an older beta!!!
4. Flash the Rom and Gapps.
5. Reboot

If you have already an samsung 4.1.x running you can skip step 1
1.flash N7000ZSLR1_N7000OZSLR1_BRI (or any other other 4.1.2) stock rom using odin
2.flash PhilZ-cwm6-XXLT6-XSA-5.06.1.tar.md5 (http://d-h.st/OTu) to get cwm recovery by using odin or mobile odin
3. boot into recovery
4. flash this kernel https://www.androidfilehost.com/?fid=95887005526788121 to get an updated cwm recovery
(this step will make your old rom unbootable stuck in the n7000 screen but you can boot into recovery with press and hold "home" and "volume+" buttons and power on your phone)
5. you need to reboot! your device now to recovery
6. you are now in the updated recovery and you can now install the rom and gapps and do an factory reset
7. Reboot
注意 :
If you want to use another kernel make sure that it"s ready for CM12.1 Beta 1/2/3/4/5 (Do not use older kernel for CM12.0 in the CM12.1 Betas! Kernel for CM12.1 Preview/Beta 1/2/3 should work in CM12.1 Beta 5 too, but do not use the Beta4 kernel in Beta5! )
現在已有高手提供這個不錯的 raw kernel 能讓 N7000 4.1.2 直刷CM12.1及CM11, omni 等,它的優點有安全內核可以wipe及有充電快速等..

刷機前先備份EFS及資料,取出SIM卡等, 如果是在官方最新版本4.1.2 的話 , 請記得先刷安全內核PhilZ-cwm6-ZSLO2-OZS-5.08.5.tar.md5
1.Download Raw kernel for cm12.1 (r10 for CM12.1) & CM12.1 beta5 rom and Android 5.1 Google apps
2.put all the .zip files in your SDcard
3.Flash raw kernel and reboot recovery
4.Wipe data/factory reset , cache partition & dalvik cache , format system & Preload
5.install the rom & google apps
ps.請勿在 anroid 2.3.5 或 4.0.X 刷 PhilZ-cwm6 5.08.5 或 raw kernel r10 等 , 否則會變磚哦
